Prenatal Care Guidance - PCG

Prenatal Care Guidance (PCG) is a public health program that works closely with Healthy Start and WIC to help patients prepare for childbrith, support young and inexperienced mom's, new immigrants, and women who have a limited social support. They only accept referrals while women are pregnant. They utilize community health workers that supports and provides comprehensive patient education to women who are pregnant and follows them through 1 year postpartum.

Assists with:
  • Medi-cal application/presumptive eligibility
  • Substance abuse/Mental health education and referral
  • Social services outreach and navigation (housing, food stamps, etc)
  • Education on bonding and attachment
  • Postpartum depression screening
  • Child development screening (ages and stages questionnaire)

Appropriate referrals are women who:
  • are new to the area or who are not conected to services
  • are teens
  • are pregnant for the first time
  • have gestational diabetes
  • have Potential anomalies
  • are isolated, with poor social support network
